Job Title
Physical Therapist at Harbor Health & Rehab
Job SummaryConsulting with residents to learn about their physical condition and symptoms; diagnosing movement dysfunction and developing a treatment plan; teaching residents how to effectively use therapeutic exercise techniques.
Responsibilities- Responsible for evaluation and treatment of residents with musculoskeletal deficiencies, neurological conditions, balance disorders, and transfer/gait deficits, after receiving a written order from the physician.
- Screening of residents: new facility residents within 24 hours of admission; long‑term residents upon receipt of a "Significant Change" or referral from nursing.
- Complete documentation: initial evaluation forms, weekly progress notes, monthly recertification forms, discharge summaries, and other required forms as designated by the facility and Regional Manager.
- Attend all required facility meetings and in‑services, including Medicare meetings, resident staffing, family meetings, and care plans.
- Co‑develop programs with the Regional Manager to meet the needs of residents and help them achieve their highest feasible level of functioning.
- Assist the facility in meeting state, federal, and other certification standards.
- Follow facility infection‑control policies and procedures and respect OBRA Resident Rights Guidelines.
- Schedule residents on the caseload appropriately and attend all required in‑services and meetings.
- Contact the Regional Manager if productivity levels consistently fall below or exceed the standard to indicate the need for additional staffing or assistance with screening.
- Schedule and conduct in‑services for facility staff education.
- Present oneself professionally, including conduct, communication, and appropriate dress while in the facility.
- Perform other duties as assigned by the PT Director and facility.
Reports directly to the Regional Manager who oversees clinical skills, documentation, and compliance with professional standards. PT assistants are indirectly supervised by the RPT and carry out treatment plans as established by the RPT, with all documentation signed by the RPT.
Physical DemandsThe role involves moderate‑heavy work demand, including lifting and transferring residents to and from various surfaces, with maximal assistance for residents weighing over 100 lbs. Occasional transfer of supplies up to 50 lbs to chest level and 10 lbs overhead. Push/pull activities involve residents up to 300 lbs in wheelchair distance up to 200 yards. Work is performed in varied environmental conditions with specified position tolerance for non‑material handling and postures.
Education/ Qualifications
- Must read, write, and understand English.
- Must attend a minimum of 12 hours of continuing education programs annually to maintain certification.
- Bachelor of Science in Physical Therapy and state licensure.
- Knowledge of therapeutic techniques.
