• Provide direct patient care, including administering medications, wound care, and vital sign monitoring
• Develop and implement individualized care plans in collaboration with the interdisciplinary team
• Communicate effectively with patients, families, and healthcare team members to ensure seamless transitions and continued care
• Participate in quality improvement initiatives to improve patient outcomes and satisfaction
• Collaborate with physicians and other healthcare professionals to ensure accurate and timely care
