Saint Elizabeth Home, a 168‑bed skilled nursing and rehab center, is currently hiring an RN MDS Coordinator. The RN MDS Coordinator schedules, coordinates, directs, oversees, and participates in the completion of the Resident Assessment Instrument (MDS, RAPS, CARE PLANNING) in accordance with federal and state regulations, and standards set by the Joint Commission. The coordinator ensures that MDS and RAPS are completed accurately and that care plans evolve from comprehensive multidisciplinary assessments.
Qualifications- Must be a Registered Nurse licensed by the state of Rhode Island.
- Must have a minimum of one (1) year of experience as a supervisor in a hospital, long‑term care facility, or related health‑care facility.
- Must have 2‑3 years of geriatric nursing experience.
