Hartford Health Care at Home is seeking a Hospice Registered Nurse to provide skilled nursing care to clients in their homes and community settings in the Mansfield region. Candidates with CT licensure and at least one year of nursing experience, including hospice or home care, are preferred.
The role involves comprehensive assessments, developing and updating care plans, and coordinating with the interdisciplinary team to ensure compassionate, outcome-based care for patients at end of life.
