Hospice Chaplain

As a Hospice Chaplain with John Paul VNA Hospage, you will have the opportunity to make a profound impact on the lives of patients and their families as they navigate the end-of-life journey. Our Hospice Chaplain provides spiritual care and support to patients and families, enhancing their overall well-being and quality of life.

• Provide spiritual care and support to patients and families, addressing their emotional, social, and spiritual needs

• Conduct spiritual assessments and create individualized spiritual care plans in collaboration with the interdisciplinary team

• Lead and participate in various spiritual activities, such as prayer services, meditation, and worship services

• Offer emotional support and counseling to patients and families, facilitating open communication and facilitating grieving processes

• Collaborate with the hospice team to ensure comprehensive care, addressing physical, emotional, and spiritual needs

• Develop and implement spiritual care plans that align with the patient's values, beliefs, and preferences

• Provide bereavement support to patients and families, helping them cope with their loss

• Facilitate spiritual growth and development through education and support

• Maintain accurate and timely records of spiritual care services

• Participate in in-service training and education programs to enhance knowledge and skills
