Overview
Center for Human Development (CHD) is currently looking for Overnight Behavioral Health LPNs to join the team at Heron House, an Enhanced Medical GLE program. This brand new 18-bed facility located in East Longmeadow, MA offers a small LPN to client ratio under the supervision of a designated charge nurse or RN with a support team of LPNs, CNAs and Direct Care staff, providing care and compassion while gaining valuable experience in CHD’s modern EMGLE facility.
A LITTLE ABOUT THE EMGLE FACILITY:
The Enhanced Medical GLE (EMGLE) provides a variety of skilled healthcare and supportive services including around-the-clock nursing and personal care assistance to Persons with Complex mental illness in addition to medical needs. These services are designed to meet and support the daily needs of Persons with chronic medical conditions, terminal illnesses and/or disabilities, which are impacted by their significant mental illness.
These Persons require licensed and unlicensed professional services to meet their chronic complex medical and behavioral needs to ensure improvement or stabilization of their medical and behavioral condition. This model will also serve to maintain, prevent, or slow worsening of condition as well as assisting transition to a lower level of care setting or prevent hospitalization/nursing facility admissions.
We currently have full-time overnight 3rd shift openings available.
A competitive hourly wage of $38.50/hr is being offered, with a $2.00/hr differential for overnight shifts. Additional holiday bonus pay provided for working on New Year s Eve, Fourth of July, Labor Day, Memorial Day and Christmas Day. YOUR ROLE AS A LPN
- Medication administration and management. Assist in medication audit processes
- Utilize CHD’s Safety Care training skills to successfully deescalate and redirect participants in time of crisis.
- Attend participants care plan meetings/resource meetings
- Provide direct care as needed. Providing personal hygiene care by assisting with travel to the bathroom; helping with showers and baths
- Need to effectively understand details about a patient’s health and provide charge nurse or RN with clear, precise information and observations
- Provide activities of daily living by assisting with serving meals, feeding Persons as necessary; ambulating
- Provide adjunct care by administering nonsterile dressings, surgical preps, ice packs, heat treatments
- Maintains the stability of Persons served by checking vital signs and weight; recording intake and output information.
Qualifications:
- Have a license in practical nursing by The Board of Registration in Massachusetts
- One-year experience as a LPN, preferred
- One-year experience in a Behavioral Health setting, preferred
- One-year experience working with the elderly, people with disabilities, behavioral challenges and cognitive disabilities, preferred
- Preferred are those candidates that have successfully completed Safety Care Training that utilizes physical restraints, de-escalation and redirection techniques. (Training provided).
- Valid driver’s license.
- Meeting physical demands of the role. IE:
Ability to lift 45 lbs or more, stocking supplies, extended periods of walking and/or standing - Participate and successfully complete assigned training requirements in compliance with CHD and the Massachusetts Department of Mental Health
