Heron House is a brand-new 18-bed Enhanced Medical Group Living Environment (EMGLE) program. As a Licensed Practical Nurse, you will provide skilled nursing and personal care to persons with mental illness, chronic medical conditions, terminal illnesses, or disabilities, under the supervision of a charge nurse or RN and a support team of LPNs, CNAs, and direct care staff.

• Medication administration and management.
• Assist in medication audit processes.
• Attend participant care plan and resource meetings.
• Provide direct care, including personal hygiene and assistance with bathroom travel, showers, and baths.
• Collaborate with charge nurse/RN to provide clear, precise information and observations.
• Assist with activities of daily living: serving meals, feeding, ambulating.
• Administer nonsterile dressings, surgical preps, ice packs, heat treatments.
• Monitor vital signs, weight, intake and output; record data.
• Participate in the monthly on‑call rotation (stipend provided, additional wage if called in to pass meds).

• Licensed Practical Nurse in Massachusetts.
• One‑year experience as an LPN (preferred in a behavioral health setting, elderly care, or with people with disabilities, behavioral challenges, and cognitive disabilities).
• Valid driver’s license.
• Physical ability to lift 45 lbs or more, stand/walk for extended periods, and handle stocking supplies.
• Completion of required training in compliance with CHD and Massachusetts Department of Mental Health policies.
• Associate’s degree or higher in Nursing or a related field.

Competitive hourly wage of $38.50 per hour, with additional holiday bonus pay for working on New Year’s Eve, Fourth of July, Labor Day, Memorial Day, and Christmas Day. Attractive benefit package available.
