Job Summary

The Transitions of Care (TOC) Clinical Pharmacist plays a critical role in supporting the safe, effective, and seamless movement of patients across care settings, particularly during hospital admissions and discharges. This pharmacist works as part of a dedicated team of TOC pharmacists and collaborates closely with interdisciplinary healthcare providers—including discharge planners, nursing coordinators, case managers, and the patient experience team—to conduct medication reconciliations and optimize medication-related care.

This position also works in partnership with the outpatient services to support the health system, coordinating medication orders, delivery, and patient counseling le the primary focus is on transitions of care, the TOC pharmacist will also provide inpatient pharmacy services, particularly during scheduled holidays, weekends, and potential evening shifts to support operational continuity.

Pharmacists practice within a progressive clinical model at Care Well Health Medical Center, engaging in clinical monitoring, sterile and non-sterile compounding, medication distribution, and direct patient education. As part of an academic medical center, pharmacists are encouraged to participate in teaching, APPE rotations, and professional development while practicing at the top of their license.

Key Responsibilities
  • Deliver comprehensive clinical pharmacy services including assessment, monitoring, and optimization of individualized pharmaceutical therapy.
  • Perform medication reconciliation at admission and discharge in collaboration with providers, ensuring continuity and accuracy of medication regimens.
  • Support the Meds to Beds program through coordination of discharge medication orders, delivery logistics, and patient counseling.
  • Provide post-discharge follow-up phone calls and medication assessments for enrolled TOC patients.
  • Act as a clinical resource to healthcare professionals, offering education and expertise on medication management in transitions of care.
  • Serve diverse patient populations including Behavioral Health, Withdrawal Management, and geriatrics.
  • Monitor and evaluate medication use in high-risk patient populations to prevent readmissions and adverse drug events.
  • Build strong, collaborative relationships with prescribers, advanced practice providers, nursing staff, and care coordination teams.
  • Assist in the development, implementation, and adherence to protocols and policies related to discharge planning and medication use.
  • Provide drug monitoring services and medication-related recommendations to ensure safe and evidence-based therapy.
  • Maintain compliance with institutional policies and pharmacy laws; participate in documentation, quality assurance programs, and departmental training initiatives.
  • Prepare and dispense medications for inpatient use, including sterile and non-sterile compounded products.
  • Fulfill pharmacist duties in various practice settings including inpatient units, emergency departments, and transitions of care programs.
Other Duties

In addition to TOC responsibilities, the pharmacist may be required to support inpatient pharmacy operations as needed. This may include covering inpatient shifts due to staffing challenges, shortages, PTO coverage, or other operational needs. Flexibility and the ability to float between TOC and inpatient responsibilities are essential to ensuring continuity and quality of pharmacy services.

Minimum Education & Certifications
  • Doctor of Pharmacy (Pharm

    D) from an accredited College of Pharmacy
  • Current and Active New Jersey Board of Pharmacy Pharmacist License
  • National Provider Identifier (NPI) number is required for credentialing with insurance carriers.
  • BLS for Healthcare Providers certification (required)
Preferred Experience
  • Minimum of 1 year of inpatient hospital pharmacy experience (preferred)
  • Completion of a PGY1 pharmacy residency program in a relevant clinical area or equivalent experience (preferred)
Physical Demands Analysis
  • Frequently required/prolonged sitting, standing, or walking.
  • Manual and finger dexterity to operate computer & equipment.
  • Walking, standing, and lifting of materials/equipment up to 60lbs.
  • Visual acuity required to read written and printed materials.
  • Work performed in a climate-controlled, indoor setting.
