Job Overview
Provide supportive services to participants in the Foundational Community Supports (FCS) program. Deliver housing services to execute Individual Housing Plans, enhancing housing security for eligible participants through direct and indirect service provision at Goodwill and partner agencies. Must pass driver record and background check.
Education & Experience- Bachelor's degree in human or social services from an accredited college or university and at least two years of demonstrated experience providing supported employment or similar services; OR certified employment support professional (CESP); OR certified rehabilitation counselor (CRC); OR Associate degree with three or more years of demonstrated experience providing supported employment or similar services.
- Assist participants in achieving housing stability.
- Conduct housing needs assessments, locate and obtain stable housing, monitor transition to housing, and ensure participants receive necessary supportive services to maintain housing.
- Establish and document participant housing requirements, determine household barriers, gather and record prior housing information to coordinate effective housing searches, locate landlords willing to rent, perform inspections, and complete other housing-related tasks.
- Coordinate or provide supportive services needed to achieve and maintain stable housing.
- Comply with State and Federal Fair Housing Laws.
- Use informal community networks to locate housing and make referrals as needed to support Individual Housing Plans.
- Participate in meetings, staffings, and other activities to promote and review the effectiveness of Individual Housing Plans.
- Explain and provide written information on the coordinated entry process to eligible and ineligible participants to access resources.
- Conduct other services and activities in concert with the Foundational Community Supports program guide and at the direction of the Goodwill Program Manager.
- Assist with the development and implementation of programs and services.
- Assist with screening of referrals for appropriate program and service options.
- Establish and maintain good working relationships with participants, referral agencies, landlords, and all team members.
- Follow up on participant progress, providing encouragement as needed.
- Ensure all paperwork and related activities are current, accurate, and in compliance, including case notes and required documentation.
- Enter required data into the Participant Case Management database in a timely and accurate manner.
- Provide services in accordance with Goodwill policies, CARF standards, and principles of safety.
- Assist participants in achieving the highest level of independent functioning while receiving services.
- Facilitate Person-Centered Planning to determine housing goals, recommend appropriate services, oversee program and goals, and revise as necessary.
- Model appropriate work behaviors and interpersonal skills.
- Inform business, labor, and the general public about Goodwill services and potential employees.
- Perform other duties as assigned.
