Position: Residential Counselor: Transitional Support Program - Part Time- Second Shift!- 6726

Residential Counselor – Transitional Support Program (Part Time, Second Shift)

Community Health Resources invites applicants to join our Transitional Support Program (TSP) located in East Windsor, CT. The program is a Supported Apartment Program for young adults ages 18‑25 and is staffed 24/7 by a multidisciplinary team.

Duties & Responsibilities
  • Maintain a safe, clean, and program‑standard physical environment in compliance with licensing requirements.
  • Administer and monitor medications for all residents following the Adult Residential Medication policy.
  • Effectively communicate daily event information to staff and team members.
  • Teach and assist residents in daily living skills such as nutrition, healthy living, meal preparation, hygiene, laundry, banking, budgeting, and basic home‑care skills.
  • Assist with administrative tasks and functions as directed by the supervisor.
  • Prepare and facilitate educational groups and one‑to‑one interventions per treatment plan.
  • Collaborate and coordinate care with providers, families, and program staff.
  • Assist residents in addressing medical and psychiatric needs.
  • Maintain accurate documentation in compliance with agency and program standards.
  • Participate in staff meetings, supervision, trainings, and other meetings as assigned.
  • Perform additional duties as required.
Qualifications
  • High school diploma or GED required.
  • Preferred:
    Two years’ experience working with a relevant young adult population.
  • Valid driver’s license required with a good driving record.
