Residential Counselor – Transitional Support Program (Part Time, Second Shift)
Community Health Resources invites applicants to join our Transitional Support Program (TSP) located in East Windsor, CT. The program is a Supported Apartment Program for young adults ages 18‑25 and is staffed 24/7 by a multidisciplinary team.
Duties & Responsibilities- Maintain a safe, clean, and program‑standard physical environment in compliance with licensing requirements.
- Administer and monitor medications for all residents following the Adult Residential Medication policy.
- Effectively communicate daily event information to staff and team members.
- Teach and assist residents in daily living skills such as nutrition, healthy living, meal preparation, hygiene, laundry, banking, budgeting, and basic home‑care skills.
- Assist with administrative tasks and functions as directed by the supervisor.
- Prepare and facilitate educational groups and one‑to‑one interventions per treatment plan.
- Collaborate and coordinate care with providers, families, and program staff.
- Assist residents in addressing medical and psychiatric needs.
- Maintain accurate documentation in compliance with agency and program standards.
- Participate in staff meetings, supervision, trainings, and other meetings as assigned.
- Perform additional duties as required.
- High school diploma or GED required.
- Preferred:
Two years’ experience working with a relevant young adult population. - Valid driver’s license required with a good driving record.
- Competitive hourly rate starting at $17.00/hr plus commensurate pay based on experience.
