Salary: £50,000 - £65,000 per annum

We are seeking a highly motivated and skilled Advanced Nurse Practitioner or Advanced Clinical Practitioner to join a forward-thinking, multidisciplinary primary care team. • Provide advanced clinical care for patients with acute and chronic medical conditions.
• Deliver evidence-based care in line with NICE guidance and professional standards.
• Identify and support patients at risk of long-term conditions.
• Conduct complex chronic disease clinics, wound care, and other specialist interventions.
• Work autonomously and collaboratively within a multidisciplinary team.
• Mentor and support junior staff and students.
• Maintain accurate clinical records and contribute to audits and quality improvement initiatives.
• Participate in health promotion and preventative care programs.

• Registered Nurse with current NMC registration or HCPC or GHPC.
• Independent Nurse Prescriber (desirable).
• Master's degree or post-graduate qualification in Advanced Practice (Desirable).

• Experience in primary care and general practice.
• Demonstrable advanced nursing practice across clinical, leadership, education, and research domains.
• Experience of prescribing and undertaking medication reviews.

• Strong clinical assessment, diagnostic, and decision-making skills.
• Proficient in acute and chronic conditions.
• Knowledge of public health issues and evidence-based practice.
• Excellent communication, team working, and mentoring skills.
• Ability to work under pressure, prioritise workload, and maintain professional integrity.

• DBS clearance and occupational health clearance.
• Flexibility to work outside standard hours if required.
