Reports to the Rehabilitation Manager, Lead Physical Therapist, and/or staff Physical Therapist.

• Treat residents as directed by the Physical Therapist and contribute to effectiveness by identifying patient care issues and providing information and commentary.
• Monitors patient progress throughout treatment; prepares and maintains records and reports as appropriate, ensuring adherence to procedural safeguards and confidentiality requirements.
• Documents patient care services by charting in patient and department records.
• Participates in resident care conferences and weekly rehabilitation meetings.
• Communicates with supervisor and other interdisciplinary team members regarding resident progress, problems, and plans.
• Participates in facility in-service training programs.
• Records daily treatments and labor according to corporate procedure.
• Instructs families and staff in maintenance programs as directed by the Physical Therapist.
• Practices safety, environmental, and infection control methods.
• Familiar with Standard Precautions, Exposure Control Plan, Fire Drill and evacuation procedures and knows how to use information.
• Protects residents from neglect, mistreatment, and abuse.
• Assists with cleaning and maintenance of treatment area and department.
• May require work beyond normal duty hours, on weekends, and temporary other positions when necessary.

• Associate's degree.
• Current license in state of practice.
• Certification, if applicable, and demonstrated competency in identified areas of specialization.
• Organized and detail-oriented work performance.
• Good communication skills with excellent self-discipline and patience.
• Genuine caring for and interest in elderly and disabled people in a nursing facility.
• Compliance with Residents’ Rights and Facility Policies and Procedures.
