Position Summary
The Bereavement Coordinator plans, implements, and maintains a comprehensive bereavement program to support families and caregivers for up to one year following the death of a hospice patient. This role also provides grief support to hospice staff and serves as a resource for the community.
Key Responsibilities
- Coordinate grief counseling services (individual, group, and follow-up) for families and caregivers.
- Develop and evaluate the Bereavement Program annually for quality and effectiveness.
- Provide emotional support to hospice staff and supervise counseling interns.
- Offer bereavement information and referrals to community members.
- Maintain accurate, confidential records and ensure compliance with Medicare Hospice Conditions of Participation.
- Participate in hospice activities, education, and quality improvement initiatives.

Requirements
- Active Maryland Licensed Certified Social Worker - Clinical: LCSW-C
- Ability to work as a team member.
- Knowledge and commitment to hospice philosophy.
- Demonstrates good verbal and written communication, and organization skills.
- Understanding of grief response and experience with counseling individuals and/or in groups regarding loss.
- If making home visits, must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
