DescriptionAs an Admissions Nurse, you will prioritize the needs of the patient and family, ensuring a smooth transition into hospice care. You will evaluate patient eligibility, conduct assessments, and collaborate with healthcare providers, managing multiple admissions and documentation efficiently. Key Responsibilities- Conduct timely admissions assessments in homes, facilities, or hospitals.
- Explain hospice philosophy, services, and benefits to patients and families.
- Evaluate patient eligibility for hospice services based on clinical criteria.
- Collaborate with physicians, case managers, and other healthcare providers.
- Initiate and complete all required documentation accurately and efficiently within EMR systems (eg. MyUnity/Netsmart).
- Ensure a smooth transition into hospice care with compassion and professionalism.
- Maintain compliance with all regulatory and organizational standards.

- Graduate of an accredited school of nursing.
- One to two years of recent critical care experience in an institutional setting, and one to two years of recent experience in home care.
- Current RN licensed in State of Maryland and CPR certification.
- Bachelor’s degree, with one year of hospice care experience preferred.
- Strong clinical assessment and communication skills.
- Compassionate demeanor and commitment to patient-centered care.
- Ability to work fast-paced, independently and efficiently in diverse settings.
- Prolonged or considerable walking or standing; able to lift, position and/or transfer patients; lift supplies and equipment; reach, stoop, bend, kneel, or crouch as needed; visual acuity and hearing to perform required nursing skills.
- Must be a licensed driver with an automobile that is insured in accordance with state/organization requirements and is in good working order.
