Description
The Registered Nurse Case Manager plans, organizes and directs hospice care and is experienced in nursing, with emphasis on community health education/experience. The Registered Nurse professional builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.
Key Responsibilities
- Complete initial and ongoing comprehensive assessments of patients and families
- Develop, implement, and revise individualized plans of care
- Serve as the primary case manager for assigned patients, coordinating all aspects of care
- Administer medications and treatments per physician orders
- Provide education, counseling, and support to patients and families
- Promote patient independence, comfort, and quality of life
- Maintain accurate, timely clinical documentation
- Communicate changes in patient condition to physicians and interdisciplinary team
- Collaborate closely with hospice team members to ensure continuity of care
- Provide patient and caregiver education regarding care, medications, and resources
- Participate in interdisciplinary group (IDG) meetings
- Participate in on-call rotation as assigned
- Coordinate equipment, supplies, and services for patient care
- Supervise and delegate tasks to hospice aides as appropriate
- Engage in ongoing professional development and continuing education
- Participate in quality improvement activities
