Job Description:

Under direct supervision, provides a broad range of paraprofessional social services to clients and their families. Provides information to clients, conducts intakes or assists staff in the delivery of social services.

This position has designated special entry salary and is non-negotiable.

Role and Responsibilities:

Assists clients in locating and utilizing community resources including legal, medical, financial assistance, and other referral services.

Completes patient/client/consumer intakes based on interviews with patient/clients/consumers, their families, significant others and appropriate community agencies.

Maintains contact with other social service agencies and healthcare providers involved with clients to provide information and obtain feedback on client’s overall progress.

Organizes the delivery of specific social services within the community.

Prepares intake reports and history records.

Minimum Qualifications High school diploma/GED AND One (1) year of job-related experience in a social services related position.

Preferred Qualifications:

Preference will be given to candidates who, in addition to meeting the minimum qualifications, demonstrate some or all of the following skills/experience:

Prior experience working in a child welfare related field.
