• Provide professional nursing care in patients’ homes, assessing, planning, implementing and evaluating individual care continuously.
• Ensure compliance with physician orders, timely documentation, and progression toward discharge goals.
• Follow policy, procedure and standard of care for assessment, planning, intervention and evaluation; provide holistic care to patient and significant others; coordinate interdisciplinary care.
• Apply clinical judgment, critical thinking and ethics reflected through accurate EMR documentation.
• Identify and comply with payer authorizations, maintain visit frequencies, and communicate with insurance nurses.
• Update care plan within two weeks of any change in patient status.
• Implement teaching strategies that meet plan of care goals; document strategies in chart.
• Report patient care problems and use managers as resources for solutions.
• Act as liaison among physicians, managers and team members for effective coordination and referrals.
• Communicate patient information during transfers and scheduling of vacations or time‑off.
• Complete routine assignments independently and meet productivity requirements.
• Maintain awareness of updated standards and incorporate into practice.
• Perform other duties or projects as assigned; demonstrate flexibility.
• Adhere to HMH organizational competencies and standards of behavior.
• Lift a minimum of 50 lbs, push/pull 50 lbs, and stand a minimum of 8 hours daily.

• Associate’s Degree in Nursing or Diploma/Certificate in Nursing.
• Minimum of one year RN experience.
• Ability/experience with inserting IVs.
• Competency with IV experience: within one year required; demonstrate insertion of peripheral line, ports, PICCs, and central lines with preceptor.
• Upload ONS/ONCC Chemotherapy/Immunotherapy certificate within three months of hire (agency‑paid).
• Successful completion of infusion test during probation with 80 % or higher.
• Excellent written and verbal communication skills.
• Proficient computer skills (Google Suite, Microsoft Office).

• BSN

• NJ State Professional Registered Nurse License.
• AHA Basic Health Care Life Support (HCP) Certification.

Minimum hourly rate of $42.52. Pay honors transparency and aligns with the New Jersey Pay Transparency Act. Base compensation may vary based on market data, experience, education, certifications, skills, location, internal equity, and grant funding.
