• Graduate of an accredited school of nursing; 1-2 years of recent acute care experience in an institutional setting; 1-2 years of recent experience in home care preferred
• Current licensure in State
• Management experience not required; responsible for supervising hospice aides
• Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist
• Prolonged or considerable walking or standing; able to lift, position and/or transfer patients; able to lift supplies and equipment; visual acuity and hearing to perform required nursing skills
• Must be a licensed driver with an insured automobile in good working order

• Complete initial comprehensive assessment of patient and family to determine hospice needs; provide complete physical assessment and history of current and previous illness
• Provide professional nursing care by utilizing all elements of nursing process; assess and evaluate patient’s status by writing and initiating plan of care
• Regularly re-evaluate patient and family/caregiver needs; participate in revising the plan of care as necessary
• Initiate appropriate preventive and rehabilitative nursing procedures; administer medications and treatments as prescribed by the physician
• Counsel the patient and family in meeting nursing and related needs; provide health care instructions as appropriate per assessment and plan
• Assist the patient with activities of daily living and facilitate patient’s efforts toward self-sufficiency and optional comfort care
• Act as RN Case Manager when assigned by Clinical Supervisor; assume responsibility for coordinating patient care for assigned caseload
• Communicate with physician regarding patient’s needs and report changes; obtain/receive physician orders as required
• Communicate with community health related persons to coordinate care plan; teach patient and family/caregiver self-care techniques as appropriate
• Provide medication, diet and other instructions as ordered by physician; recognize and utilize opportunities for health counseling with patients and families/caregivers; work in concert with interdisciplinary group
• Maintain safe environment for patient; assist patient and family/caregiver and other team members in providing continuity of care; attend interdisciplinary group meetings
• Participate in on-call duties as defined by on-call policy; ensure arrangements for equipment and other necessary items and services are available
• Supervise ancillary personnel and delegate responsibilities when required; assume responsibility for personal growth and development; maintain and upgrade professional knowledge and practice skills through continuing education and in-service classes
• Actively participate in quality assessment performance improvement teams and activities
