Summary of Position:

  • Provides skilled hospice nursing care to patients in collaboration with Registered Nurse Case Manager. This may include but not limited to routine visits, admissions, continuous care, death visits etc. Provides services in accordance with the Hospice’s philosophy, standards and polices.
Work Environment/Physical Requirements
  • The responsibilities of this position involve significant physical activities including standing, lifting, bending, stooping, pushing, pulling and twisting.
  • The employee must be a licensed driver with an automobile that is in good working order and insured in accordance with the organization requirements.
  • The employee may experience a change in work conditions related to weather issues, traveling in rain, snow, etc., and must be able to adapt to the work environment to meet patient care needs.
  • The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job.
  • While performing the duties of this job, the employee is regularly required to talk or hear. The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.
Requirements of the Position:

Required Education and Experience

  • Graduate of an accredited school of nursing. One (1) year of recent acute care experience in an institutional setting.
  • Current licensure in state.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.

Required Skills, Knowledge, and Abilities

  • Able to relate positively, effectively, and appropriately with patients, families, community members, and providers.
  • Able to read, write, speak and understand English.
  • Meets all health requirements, as required by law.
  • Demonstrates basic computer knowledge and ability with an aptitude to learn company applications.
