Are you an experienced Social Worker passionate about supporting adults to achieve their goals and maintain independence? We are recruiting for a Social Worker to join one of Newport’s NCN Managed Care Teams, delivering high-quality, strengths-based support across the community.
This is an excellent opportunity to work with adults of all ages, completing assessments, reviews and short-term interventions that promote wellbeing, choice and independence.
The Role
Based in Newport, South Wales, you will work from the Civic Centre and within the community, supporting adults in their own homes, residential/nursing placements and supported living environments.
You will work collaboratively with individuals, families, carers and partner services including mental health, learning disability and physical disability teams to deliver effective care and support in line with the Social Services and Well-being (Wales) Act 2014.
Key Responsibilities
* Manage a caseload of adults with a range of care and support needs.
* Complete strengths-based assessments, reviews and care planning.
* Undertake risk assessments and develop appropriate risk management strategies.
* Support individuals to maximise independence and achieve positive outcomes.
* Work closely with families, carers and multi-disciplinary professionals.
* Maintain accurate case records and electronic systems.
* Produce clear, high-quality assessments and reports.
* Deliver short-term interventions to promote wellbeing and reduce dependency.
* Work flexibly across community settings to meet service needs.
About You
You will have:
* A recognised Social Work qualification and relevant registration.
* Experience with in statutory adult social care (desirable).
* Strong assessment, communication and risk management skills.
* Experience applying strengths-based approaches.
* Ability to manage a caseload and prioritise competing demands.
* Excellent written and verbal communication skills.
* Experience working collaboratively with families, carers and partner agencies.
* A flexible and proactive approach.
* A valid driving licence and ability to travel within the community.
