Starting Pay: $35.00 - $45 per hour based on experience.

Homewatch Care Givers of Eden Prairie is seeking a full time Home Care Nurse! It is our priority to ensure every employee of Homewatch Caregivers receives the respect and recognition they deserve! • Perform new client assessment/admission
• Perform quality assurance and supervisory visits
• Communicate with necessary people to ensure proper execution of client care plans (employees, clients, family members, etc.)
• Maintain professional relationships with the clients, caregivers, and all internal and external people involved in client care team.
• Provide care for clients in their home environments, building meaningful relationships by helping them achieve their goals to remain healthy at home
• Communicate with physicians, other healthcare providers, and support staff to ensure continuity of care.
• Monitor caregiver compliance with company policies and client care plans
• Administer medications and treatments as prescribed.
• Assists with employee training during on-boarding and throughout caregiver tenure
• Provide wound care and other specialized nursing interventions
• Oversee and carry out all the nursing responsibilities involved with maintaining a comprehensive Home Care license with the Minnesota Department of Health.
• Assist our office with State MN Department of Health Survey.

• Registered Nurse in MN
• 3+ years of recent experience
• Excellent customer service and communication skills
• Ability to function with minimal supervision
• Must maintain nursing license and required skills to perform the job and adhere to the Agency policies
• Home care, home health or hospice, and/or leadership experience (Preferred)
