In this role, you will primarily be creating and managing a plan of care for individual patients. You are a Licensed Practical Nurse (LPN) who operates with great attention to detail and a big heart. You take pride in creating personalized care plans for patients and are able to observe and communicate effectively in your patient's best interest. To succeed, you should have demonstrated knowledge and skill in current nursing practices as well as a strong penchant for solving problems.
Responsibilities- Performs focused nursing assessment of Residents to determine appropriate interventions to maintain or improve the Resident’s health status.
- Provides interventions for Residents with episodes of potentially life‑threatening instability.
- Provides nursing visits and direct care to Residents in the resident's home or assisted living as directed by client Care Plans.
- Sets up medications for Residents according to the medication management program, as indicated.
- Coordinates medical appointments for Residents, including follow-up with any new orders or changes after hospital visit with direction from Nursing case manager as needed.
- Accepts and implements prescriber orders with direction as needed.
- Responds to phone calls from other providers related to Residents with RN direction as needed.
- Documents, as indicated, in the clinical record.
- Communicates with families, resident representatives, case managers and other health care providers regarding the Residents as needed.
- Participates in care conferences with other members of the health care team.
- Maintains absolute confidentiality of all information pertaining to Residents, families, and employees.
- Promotes a safe and comfortable environment for the Residents.
- Demonstrates excellent written and verbal communication skills.

- Falls prevention
- Reporting of defective equipment or hazards
- Wiping spills or wet floors immediately
- Using Standard Precautions and appropriate infection control measures
- Knowledge of emergency procedures
- Reporting incidents/accidents to the Clinical Nurse Supervisor or Director on the day of occurrence
- Week 1: 7am - 7pm (Sunday)
- Week 2: 7am - 7pm (Sunday)
- Currently licensed Licensed Practical Nurse (LPN) in state of employment.
- Previous experience as a Licensed Practical Nurse is preferred.
- Excellent interpersonal and client care skills.
- Valid driver’s license.
- First aid/CPR certification required.
