Overview

- Provide care to assigned residents in a caring, safe and efficient manner according to location standards, procedures and individualized resident care plans.
- Complete comprehensive nursing assessments on admission and on an ongoing basis according to state and federal regulations using the nursing process (assessment, diagnosis, intervention, evaluation).
- Administer medications, treatments and other procedures within defined time frames and using appropriate infection control procedures; monitor for therapeutic effects and side effects of medications.
- Obtain equipment and supplies as needed; ensure procedures for pressure ulcers, dehydration, malnutrition, incontinence, pain and medication administration are followed.
- Perform glucometer testing and control monitoring as required.
- Implement approaches to address resident behavior and enhance mood as outlined in the care plan.
- Document actions and changes in resident condition according to policy; maintain accurate EMR entries and documentation for admissions, discharges, transfers, or death; complete incident reports as necessary.
- Complete behavior documentation and seven-day observation tools; participate in quality improvement processes, including audits and corrective actions.
- Assist with or complete portions of the MDS for Nursing and sign forms to signify completion when assigned.
- Ensure physician orders are processed promptly and physicians are notified in a timely manner; participate in rounds with physicians as assigned.
- Attend or obtain resident report prior to shift; communicate, observe and report residents’ conditions; respond promptly to resident call lights.
- Direct and assist with resident and family education; notify families of condition changes; follow up on signs/symptoms of infections with timely reporting.
- Graduate of an accredited school of nursing. Licensure for the state of practice is required; prior related work experience is preferred.
- Maintains required certification/qualifications and standards; knowledge of basic nursing skills and the therapeutic effects and side effects of medications.
