The Licensed Clinical Social Worker (LCSW) provides clinical social work and discharge planning services to patients and their families in collaboration with the interdisciplinary team and post‑acute care providers. Demonstrates respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.
Job DescriptionWe are seeking a Licensed Clinical Social Worker to provide comprehensive psychosocial services to patients and families in both hospital and emergency settings. Responsibilities include conducting psychiatric assessments using DSM‑IV‑TR, offering individual counseling and crisis intervention, and building strong therapeutic rapport. The role also involves identifying hospital and community resources, making referrals, and advocating for patient needs. Social workers provide psychoeducation to patients and families to strengthen coping and problem‑solving skills, while also addressing issues such as abuse/neglect, chemical dependency, grief, and discharge planning.
Additional duties include maintaining professional development through continuing education, contributing to performance improvement by evaluating patient outcomes and departmental success, and performing other assigned tasks.
- Masters of Social Work (MSW) from an accredited school by the Council on Social Work Education.
- Licensed as Licensed Clinical Social Worker (LCSW) or Licensed Independent Social Worker (LISW) in the state where work is being performed.
- Knowledge of emotional and mental disorders and the DSM.
- Excellent communication skills and the ability to coordinate care well with an interdisciplinary team.
