This is a weekend position, working 24 hours weekly and getting paid for 36 hours weekly. This position supports critical care and focuses on providing pharmaceutical care with emphasis on clinical monitoring and therapeutic interventions that improve patient outcomes.

Job Responsibilities- Provide Pharmaceutical Care to patients with an emphasis on clinical monitoring and therapeutic interventions that improve patient outcomes. Provide information and educational programs to customers. Assist with the maintenance of Performance Improvement processes to constantly improve the Medication Use Process. Assure dispensing accuracy and direct pharmacy workflow to ensure accurate and timely dispensing and distribution.
- Demonstrate respect, dignity, kindness, and empathy in each encounter with all associates, patients, families, visitors and other employees regardless of cultural background.
- Medication Order Verification:
Review and interpret medication orders from licensed prescribers or their agents to ensure completeness, appropriateness of medication and dose for the patient, assessment of drug interactions, allergy alerts, duplication of therapy, and potential therapeutic interactions. Obtain clarification from the prescriber for incomplete orders or suboptimal outcomes. Provide the prescriber with appropriate therapeutic alternatives. Perform a second verification of medication orders for pediatric patients and chemotherapy orders. - Therapeutic Clinical Monitoring:
Actively participate in ongoing monitoring of medication therapy in compliance with Collaborative Care Agreements and medication management protocols. Direct medication therapy within defined bounds, contact prescribers for therapeutic alternatives, and document assessments and recommendations in the patient’s chart. - Compounds and Dispenses Pharmaceuticals:
Ensure timely dispensing of appropriate quantities of correct medication. Supervise compounding per USP standards, verify ingredient compatibility and labeling including expiration dating. Check all medication prior to final dispensing for patient administration, distribution via automated cabinets, and floor stock dispensing. Use barcode technology where possible. Supervise repackaging of medication with documentation of lot numbers and expiration dates. - Supervise and Direct Interns and Technicians:
Provide active direct supervision and mentoring, monitor task completion, redirect staff to meet patient needs, and encourage use of the AIDET model in interactions. Report non-responsive behavior to leadership. - Medication Use Safety Initiatives:
Detect and report variances in dispensing, identify opportunities to increase safe use of medications, and report suspected adverse drug reactions. - Controlled Substance Accountability:
Participate in and supervise receipt, dispensing and distribution of controlled substances using Pyxis systems and other inventory tools. Ensure inventory accuracy and resolve discrepancies with appropriate reporting. - Contributes to Departmental Financial Health:
Encourage formulary adherence and therapeutic interchanges, process non-formulary requests appropriately, suggest alternatives for non-formulary requests, support IV to PO conversions, and aim to minimize overtime while maximizing efficiency. - Drug Information / Staff Development:
Provide accurate and timely drug information to medical and nursing staff and patients. Serve as a preceptor or advisor to Pharmacy Residents and Interns. Develop staff education programs. - Maintains Professional
Competency:
Maintain licensure and complete competency requirements. Stay current with pharmacy practice standards and pursue development in specialized areas. - Performs other duties as assigned.
- Education, Credentials, Licenses: Graduate from ACPE accredited pharmacy college;
Current US pharmacist license in the state where work is performed. - Specialized Knowledge: Problem solving and critical thinking skills; good communication skills; familiarity with Current Practice Standards (ASHP).
