* Job Summary:

** The Visit Nurse is responsible for the provision of quality care to the patient. The Visit Nurse follows the physician’s plan of treatment and the nursing plan of care. The Visit Nurse is responsible for teaching care needs to the patient, family/caregiver, and reports pertinent findings to the Clinical Care Coordinator. Demonstrate respect, dignity, kindness, and empathy in each encounter with all patients, families, visitors and other employees regardless of cultural background.

* Job Description:

** This position would be a rotation of:(2)12-hour shifts 7P - 7A one week;(1) 8-hour shift of 2P - 10P and (1) 12-hour shift 7P - 7A the following week Responsibilities:  Follows the physician’s Plan of Care for the patient, which includes physical and environmental assessment using the nursing process.  Reports to the multidisciplinary team changes needed in the Plan of Care to best meet the needs of the patient.  

Uses medical supplies as appropriate to meet the needs of the patient.  Makes decisions independently and within scope of practice regarding patient care and needs during visit.  Has the ability to adjust to the unique situations encountered in the patient’s home.  Participates in the clinical on-call rotation.  Performs assigned supervisory visits for home health aides.  Communication:  Communicates with the multidisciplinary team any changes in the information gathered throughout the nursing assessment.  

Completes documentation to support visits and completes discharge and transfer documentation.  Communicates patient care issues to the multidisciplinary team and, if appropriate, hospice leadership.  Documentation:  Documents within the EMR or downtime forms pertinent information regarding patient’s condition.  Completes documentation to support visits and completes discharge and transfer documentation.  Meets time frames required for documentation submission.  Assures documentation is accurate, complete, specific and meets all state, federal and JCAHO regulations.  

CQI/Peer Review:  Participates in Performance Improvement and chart reviews.  Education  Provide health education and community resource information to patients and families.  Maintain role as a patient health advocate while the patient is on service.  Meet all skills and competencies identified by the hospice department and healthcare system.  Performs other duties as assigned.
Education, Credentials, Licenses:  Graduate of an accredited school of nursing; licensed to practice nursing in the state where work is being performed.  Meets contact hour requirements for licensure, including all state-required courses.  Valid Driver’s license.  Specialized Knowledge:  Demonstrates advanced assessment skills appropriate for home care environment, ability to function independently; knowledge of community resources.  Kind and Length of

Experience:

2-3 years of RN experience in a hospital or care facility setting.
