Case Manager (Service Coordinator)
Service Coordination Inc (SCI) is seeking a passionate, community‑driven Service Coordinator to provide high‑quality case management to individuals with intellectual or developmental disabilities. You will collaborate with clients, families, providers, and internal teams to design and implement Person‑Centered Plans and coordinate services.

- Advocate for individuals and educate them and their families on self‑determination and services.
- Conduct comprehensive assessments and develop Person‑Centered Plans (PCPs).
- Coordinate and refer resources to meet PCP goals; maintain documentation and track progress.
- Meet with clients, families, providers; conduct on‑site visits and follow‑ups.
- Manage billing (≈75% of worked time) and travel time; submit required paperwork (RECON, AIR, waivers).
- Maintain HIPAA compliance and secure records.
- Meet performance utilization benchmarks and support SCI mission and policies.
- Collaboration, effective communication, customer focus, interpersonal savvy, trust building.
- Tech savvy, accountability, valuing differences, resourcefulness, adaptability.
- Planning and aligning with organizational goals.
- Bachelor’s degree in a human services field (or non‑human services degree with two years of relevant experience).
- Experience working with people with developmental disabilities.
Predominantly office environment; 95%+ stationary, occasional movement of 10–25 lb office equipment; ability to use computer and vehicle; good vision.
