RN Case Manager – Community-Focused Nursing Role

Location: Southwest Minnesota Region

Schedule: Full-Time (M-F, 8:00 AM – 5:00 PM), 1 weekend/month, 1 on-call shift/week

Compensation: $85,000 – $130,000 annually + $8,000 in bonuses during the first year

• Assess client needs and develop patient-centered plans of care
• Complete all required OASIS assessments and skilled documentation
• Partner with local care teams, physicians, and family members
• Educate, empower, and uplift the people you serve

• Active Minnesota RN license
• Minimum 3 years of RN experience; home care or senior care preferred
• Must have valid driver’s license and reliable transportation
