In this role, you will conduct patient assessments, perform phlebotomy and point-of-care testing, maintain accurate electronic medical records, and collaborate with healthcare professionals to promote positive patient outcomes.

Key Responsibilities
  • Perform routine patient assessments and monitor vital signs, including blood pressure, heart rate, respiratory rate, and temperature.
  • Administer oral, injectable, intravenous, and topical medications in accordance with applicable protocols and scope of practice.
  • Administer vaccines, perform phlebotomy, and conduct point-of-care testing.
  • Monitor patient symptoms, pain levels, and responses to treatment, documenting clinical findings accurately.
  • Review patient medications to identify potential side effects, interactions, and other concerns.
  • Document patient medical histories, including illnesses, medications, allergies, surgical history, and family history, in electronic health record (EHR) systems.
  • Conduct mental health screenings, including depression assessments and evaluations of mood, behavior, and cognitive function.
  • Educate patients on medication administration, self-care techniques, disease management, and available community healthcare resources.
  • Communicate effectively with patients, families, and other healthcare professionals.
  • Support patient access to appropriate healthcare services and resources.
  • Maintain a clean, organized, and well-stocked clinic.
  • Follow established clinical protocols, infection control practices, and patient safety standards.
Required Qualifications
  • Active, unrestricted Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) license in a U.S. state, territory, commonwealth, or the District of Columbia.
  • Minimum of one year of related healthcare work experience.
  • Previous clinical experience in primary care, urgent care, or an emergency room.
  • Hands-on experience with phlebotomy, vaccine administration, and point-of-care testing.
  • Proficiency with electronic medical record (EMR) or electronic health record (EHR) systems.
  • Verifiable high school diploma or GED.
Important Clinical Experience Requirement

Candidates must have relevant experience in primary care, urgent care, or emergency room settings. Candidates whose experience is limited to nursing homes, long-term care, home health, long-term acute care (LTAC), or skilled nursing facilities will not be considered.

Preferred Qualifications
  • Experience using Epic electronic medical record systems.
  • Strong patient assessment and clinical documentation skills.
  • Effective communication and patient education abilities.
  • Ability to work in a fast-paced outpatient clinical environment while maintaining accuracy and attention to detail.
Work Schedule and Environment

In-person, patient-facing position. Schedule follows clinic operating hours. Must be able to meet the scheduling requirements of the clinic.

Ideal Candidate

The ideal candidate is a clinically experienced LPN/LVN with a background in outpatient or acute care who can confidently perform clinical procedures, administer vaccines, draw blood, conduct point-of-care testing, and document patient care electronically. The successful candidate will demonstrate sound clinical judgment, strong communication skills, and a commitment to safe, efficient, patient-centered care.
