Job Description

Determines eligibility for Community Care Services Eligibility (CCSE) program services. Work involves conducting home visits, interviewing individuals who are older or have a disability, documenting information, determining need for services, developing service plans, verifying data, explaining program benefits and requirements, referring individuals for appropriate services, and assuring authorizations are registered in automated systems. Authorizes and monitors CCAD services, refers individuals to other programs, completes special assignments and reports, maintains confidentiality, verifies case information utilizing multiple automated systems, explains program benefits and requirements, adheres to all HHS and CCSE policies, and provides outstanding customer service in person, telephone, and e‑mail.

Maintains a positive and productive attitude while working in a fast‑paced environment.

Essential Job Functions
  • Attend work on a regular schedule and perform other duties as assigned.
  • Interview applicants, individuals receiving services, and resource persons; conduct telephone and/or home/site visits to assess need for services and provide information and referral to other resources. (15%)
  • Conduct reviews to determine eligibility and functional needs. (20%)
  • Obtain, verify, and calculate income and resources to determine financial eligibility for Title XX programs. (10%)
  • Maintain case record documents and enter information in an automated system. (20%)
  • Monitor services to determine if they are meeting individual needs. (15%)
  • Develop/coordinate/review service plans, authorize services appropriately, and ensure they meet individual needs. (15%)
  • Prepare basic level ongoing or special narratives or statistical reports. (5%)
Registrations, Licensure Requirements or Certifications
  • Current valid driver’s license.
Knowledge, Skills, and Abilities
  • Knowledge of interviewing techniques to obtain personal information, make inquiries, and resolve conflicting statements.
  • Knowledge of resources that serve individuals who are older or have a disability.
  • Ability to communicate orally and in writing.
  • Ability to set priorities, establish time frames, and meet deadlines.
  • Ability to establish and maintain effective relationships with individuals, coworkers, contract agency staff, and staff from other agencies.
  • Ability to operate computers and general office equipment.
Initial Screening Criteria
  • Willingness to travel at least 80% of the time.
  • Experience using email and other automated applications such as Microsoft Office.
  • High school diploma or equivalent.
Additional Information & Eligibility
  • Must have reliable transportation and be able to attend training, which may require overnight stay.
  • This position is included in a career track series and will be filled at the Worker I level. Previous employment with CCSE may be reviewed for appropriate entry level.
