A specialist community mental health nursing role supporting older adults with complex needs, dementia and risk management within a multidisciplinary Intensive Support Team.
Sussex Partnership NHS Foundation Trust is looking for a registered nurse to join the Intensive Support Team (IST) within the Wealden Neighbourhood Mental Health Team, based at Millwood in Uckfield. This is an excellent opportunity to work closely with older adults and their carers, delivering creative, evidence-based interventions that promote independence and recovery.
About the roleAs an Intensive Support Practitioner, you will hold a defined caseload and work autonomously to assess, plan and deliver specialist mental health care for older adults with complex presentations, including dementia and co-existing physical health needs. You will work collaboratively with service users, carers and partner agencies to develop personalised care plans, manage and review risk, and support safe and effective community-based care.
The role also includes providing supervision to junior staff and contributing to multidisciplinary decision-making across services.

You will be a Registered Nurse (Mental Health, Learning Disability or Adult) with significant post-qualification experience working with people with complex mental health needs. You will bring strong assessment and risk management skills, experience supervising staff, and the ability to work confidently as an autonomous practitioner within a multidisciplinary setting. A compassionate, collaborative and recovery-focused approach will be essential.
