Care Planning Coordinator

Care Planning Coordinator – Laurandy Centre, Wick – 28 hours per week (Tuesday to Friday).

Looking for an experienced Care Planning Coordinator to join our friendly and dedicated team. The role involves ensuring robust, person‑centred care planning processes comply with legislation and best practice.

Your responsibilities will include:

• Developing, maintaining, and reviewing care plans and risk assessments.
• Supporting the delivery of high‑quality, person‑centred care and support.
• Monitoring and auditing care documentation to ensure accuracy and compliance.
• Promoting positive outcomes, safety, dignity, and wellbeing for service users.
• Supporting quality assurance and continuous improvement initiatives.
• Liaising effectively with families, health professionals, and external agencies.
• Assisting with the promotion and development of the service within the local community.

• SVQ Level 4 (SCQF Level
  9) in Social Services and Healthcare, Leadership and Management, or an equivalent qualification.
• Experience in a senior care or supervisory role within health or social care.
• Excellent knowledge of person‑centred practice, care planning, and risk assessment processes.
• Strong organisational and communication skills.
• Ability to work independently, prioritise workload, and use initiative.
• Commitment to continuous improvement and delivering the highest standards of care and support.
