Charles Hunter Associates is seeking a Locum Social Worker to join a brand new Review Project under the Learning Disability Team. You will undertake Care Act reviews and reassessments for adults with learning disabilities, recording clear professional rationale.
You will conduct face-to-face visits across homes and community settings, gathering evidence from the person, families, providers, health professionals and records to inform audit-ready assessments and support plans.
