6) to join our discharge and patient flow team at Stoke Mandeville Hospital. The role focuses on coordinating safe, timely discharges from acute and community settings, with close work with multi-disciplinary teams.
The post requires knowledge of the Mental Capacity Act and Continuing Healthcare, excellent communication skills, and ability to work under pressure.
