British Red Cross is seeking a Support Worker to assist with discharge to assess in South Ayrshire and surrounding areas. You will respond to referrals from health professionals and provide practical and emotional support to vulnerable adults, helping them regain independence at home.
Site-based role with travel across the area; duties include home visits, assessing needs, creating tailored support plans, and coordinating with NHS and social care partners.
