Job Summary
Are you a passionate and experienced cardiac nurse ready to lead and innovate in heart failure care? An exciting opportunity has arisen to join the UHNM Hospital Inpatient Heart Failure Team for a 12-month period to cover maternity leave. We are seeking a dynamic and compassionate Band 7 Heart Failure Specialist Nurse to join our multidisciplinary team within secondary care. This is a pivotal role focused on delivering expert clinical care, driving service improvement, and supporting patients with complex heart failure needs.
The service delivers high quality patient‑centred care within an Ambulatory Heart Unit focusing on early diagnosis of heart failure, rapid optimisation of heart failure therapy and early facilitated in‑hospital discharge.
- Deliver expert assessment, diagnosis, and management of patients with heart failure, including medication titration and optimisation in line with national guidelines.
- Provide tailored education to patients and carers, promoting self‑management and improving quality of life.
- Lead on discharge planning and ensure continuity of care through effective communication with community and primary care teams.
- Act as a clinical leader, supporting junior staff and students through mentorship, supervision and role‑modelling.
- Contribute to the design, delivery and evaluation of heart failure services, ensuring alignment with best practice and local priorities.
- Lead or participate in audits, service evaluations and quality improvement initiatives to enhance patient outcomes.
- Deliver formal and informal training to nursing and multidisciplinary colleagues on heart failure management.
- Support or lead research initiatives and integrate evidence‑based findings into clinical practice.
- Maintain NMC registration, adhere to clinical governance frameworks and engage in continuous professional development.
