We are now welcoming applications for Community Nurse Practitioners to join our District Nursing teams across West Lothian and Edinburgh. The District Nurse provides leadership for the delivery of patient care within area of responsibility. The District Nurse has day to day managerial responsibility for the area including supervision and deployment of staff. The District Nurse will have responsibility for assessment of care needs, evaluation of care plans, ensuring the delivery of high quality care to patients. The post holder has responsibility and accountability for maintaining both clinical and staff governance.

• Edinburgh, North West District Nursing - 17 hours, Fixed Term for 12 months (CNP)
• Edinburgh, Hospital to Home Team - Full time (CNP)
