Active Care Group is recruiting an experienced Case Manager for the North East region. The role combines direct client assessment, care planning and inter-disciplinary coordination across the community.
The position is hybrid, based in Corbridge with travel to Clients across a wide geographical area; a full UK driving licence is essential. We welcome applicants who are Social Workers, Nurses, OTs, PTs, SLTs or Psychologists and offer flexible hours, DBS checks, and a supportive leadership team.
