Assessment and Enablement Officer

Teqniq is searching for a Assessment and Enablement Officer to work in the public sector. 36 hours per week. 12 week contract. 2 days office, 3 days remote.

Job Description:

We are seeking a motivated and experienced Assessment and Enablement Officer to join our Adult Social Care service. This is an exciting opportunity to play a key role in supporting adults to achieve greater independence, wellbeing and positive long-term outcomes. Through strengths-based reviews, you will work alongside people with learning disabilities, autism, mental health needs, physical disabilities and older adults to identify progress, respond to changing needs and maximise opportunities for independence and community inclusion.

Working across a range of services including home care, supported living, day opportunities, direct payments, residential care and community-based support, you will review existing care and support arrangements to ensure they remain person-centred, outcome-focused and promote prevention, wellbeing and independence. Using your professional judgement and analytical skills, you will work collaboratively with residents, carers, providers, health partners and Adult Social Care colleagues to identify what is working well, overcome barriers to achieving outcomes, and make evidence-based recommendations that support people to live fulfilling lives with the right level of support.

Key Responsibilities:

Undertake reviews of care and support arrangements across Adult Social Care services. Review care and support packages to ensure eligible needs are met and intended outcomes are being achieved. Promote strengths-based practice, supporting individuals to maximise their strengths, maintain independence and achieve outcomes that matter to them. Assess changes in needs, circumstances, risks, strengths and wellbeing through proportionate and person-centred reviews.

Work collaboratively with residents, families, advocates, providers and partner agencies to deliver positive and sustainable outcomes. Apply the principles of the Care Act 2014, Mental Capacity Act 2005 and Making Safeguarding Personal in all aspects of practice. Challenge and review existing support arrangements where outcomes are not being achieved or where alternative approaches may better meet a person's needs and aspirations.

Balance wellbeing, outcomes and ensuring resources are used effectively and proportionately. Produce clear, accurate and evidence-based review documentation, assessments and recommendations. Identify themes, emerging trends and opportunities for service improvement to support quality assurance, commissioning and best practice across Adult Social Care.

About You:

We are seeking an individual who has significant experience of working with adults with learning disabilities, mental health needs and physical disabilities, and who shares our commitment to delivering high-quality, person-centred services. You will be able to demonstrate:

Experience of reviewing adult social care packages and support arrangements. Strong knowledge of the Care Act 2014, Mental Capacity Act, safeguarding and strengths-based practice.

Experience of working with adults across a range of service settings including supported living, home care, residential care and community services. Ability to conduct robust assessments, reviews, risk assessments and support planning. Sound professional judgement and confidence in making recommendations based on evidence and outcomes. Excellent communication and relationship-building skills. Ability to challenge constructively and professionally where support arrangements may no longer be appropriate.
