The Clinical Outcomes Specialist acts as a subject matter expert for key EMR clinical workflows, ensuring all clinical staff are competent and efficient in their use. This clinical resource will partner with the Ambulatory Quality and Patient Safety (AQPS) Team. This role is also responsible for actively managing and closing gaps in care for the practice patient population, navigating a patient’s Social Determinants of Health (SDOH) opportunities through existing organizational relationships, and coordinating follow‑up appointments for preventive screenings and ongoing chronic disease management.
This position is vital to our quality improvement initiatives and directly impacts patient outcomes.
- EMR and Clinical Workflow Management:
Work closely with the assigned AQPS Practice Facilitator and act as a superuser in the clinical location. Become competent in key workflows and provide independent EPIC EMR clinical support within six months of hire. Serve as the clinic’s primary resource and subject matter expert for key quality EMR clinical workflows, conduct competency assessments, provide training, troubleshoot issues, and stay current on EMR updates. - Population Health Management and Quality Improvement:
Proactively manage and close gaps in care for targeted populations (e.g., breast cancer screening outreach, colorectal cancer screening outreach, transition of care management). Use EMR reporting and dashboards to identify patients needing intervention, track progress on quality metrics, and support the clinic’s quality improvement initiatives. - Patient Navigation and Community Resource Coordination:
Serve as a navigator for the patient population, connecting them with essential community resources related to SDOH (housing, food security, transportation). Educate patients on accessing these resources and document all navigation activities and outcomes in the EMR. - Other duties and/or projects as assigned.

Skills and Abilities
Required
- High School diploma, general equivalency diploma (GED), and/or GED‑equivalent program.
- Graduate of an accredited Medical Assistant program or LPN program.
- Minimum of 3–5 years of experience as a CMA, preferably in a hospital or ambulatory practice setting.
- Excellent written and verbal communication skills.
- Proficient computer skills, including Google Suite and/or Microsoft Office platforms.
- Experience with NCQA PCMH standards and the accreditation process.
- Experience with HEDIS, MIPS, and other value‑based care quality measures.
- Licensed Practical Nurse or Certified Medical Assistant.
- Valid driver’s license from a U.S. state.
- Patient‑Centered Medical Home Certified Content Expert.
