The Registered Nurse functions as a caregiver for a specific patient population to achieve desired outcomes. This role expands knowledge and skills, provides effective direct care as part of an interdisciplinary team, directs and oversees care provided by other caregivers through delegation, validation, evaluation, and coordination, and coordinates patient care/unit activities among nursing peers, physicians, and support services.
Responsibilities- Assessment
- Collect comprehensive data pertinent to the patient's health or situation.
- Perform age and disease‑specific assessment/data collection independently in a systematic manner focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes in patient status.
- Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Plan
- Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulate age and disease‑specific patient/family centered goals/objectives based on assessment data.
- Develop and coordinate a patient/family focused plan of care that is prioritized and encompasses the continuum of care.
- Implementation
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with patients/families, nursing colleagues, and other members of the health care team regarding patient status, rendered care, and treatment plans.
- Implement age and disease‑specific care in an organized and timely manner.
- Provide or coordinate patient/family education as needed and ensure understanding.
- Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
- Provide nursing care and treatments that reflect patients' needs, advocate appropriately, and demonstrate evidence‑based nursing practice and compliance with standards.
- Perform continuous Point‑of‑Care electronic documentation of each individual patient's assessments, plan of care, interventions, and evaluation of rendered care.
- Administer medication utilizing the newest advancements in bar‑scanning and documentation technologies designed to ensure the safest delivery of medications to patients.
- Co‑ordinate multidisciplinary discharge of patients to ensure safe transitioning and continuum of care.
- Evaluation
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills based on nursing knowledge to achieve desired patient outcomes.
- Initiate interventions related to patient's change in condition and unexpected response to care.
- Identify ethical issues in the practice setting and bring them to the attention of other team members.
- Recognize and respond to opportunities to enhance patient satisfaction.
- Acquire and maintain competencies, certifications, and continuing education to meet facility accreditation or individual licensing requirements.
- Adhere to American Nurses Association Code for Nurses with Interpretive Statements.
- Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs., and stand a minimum of 6 hours a day.
- Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
- Other duties and/or projects as assigned.
Skills and Abilities
Required
- Associate degree in Nursing or Nursing diploma.
- BSN in progress and/or willing to acquire within three (3) years of hire. (
Note:
For hires on or after November 2019 and after 6/1/2021 as per contractual agreements.) - Six (6) or more months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
- Strong ability to learn quickly, adapt to changing patient needs, demonstrate exceptional teamwork, and dedication to ongoing education.
Skills and Abilities
Preferred
- National certification in area of specialty preferred when eligible.
