Registered nurses at Hackensack Meridian Health provide patient‑centered skilled nursing care in an interdisciplinary team. This role focuses on comprehensive, evidence‑based assessment, planning, implementation, and evaluation to achieve desired outcomes for a variety of complex patients.
Responsibilities- Conduct comprehensive patient assessments, including gathering data pertinent to health status and collecting age‑ and disease‑specific information.
- Recognize clinical status changes and identify educational needs for patients and families.
- Develop individualized plans of care, formulate patient/family‑centered goals, and coordinate all aspects of the care plan.
- Communicate continuously with patients, families, nursing colleagues and other health‑care team members.
- Implement evidence‑based nursing interventions, including age‑specific care, medication administration using bar‑scanning technology, and coordinated discharge planning.
- Document all assessments, interventions, and evaluations in real‑time electronic medical records.
- Monitor progress toward outcomes and initiate appropriate interventions for changes or unexpected responses.
- Identify ethical issues and collaborate with the team to address them.
- Maintain patient satisfaction by addressing concerns and upholding standards of care.
- Adhere to the American Nurses Association code and All‑Staff RN BSN requirements.
- Perform physical requirements: lift ≥35 lbs, push/pull ≥10 lbs, and stand ≥6 hours/day.
- Associate’s or Diploma in Nursing.
- BSN in progress or willing to obtain within three (3) years of hire (or within five years under CC Union Contract).
- Minimum six (6) months of relevant nursing experience or active participation in the Hackensack Meridian Health Nurse Residency Program.
- Strong ability to learn quickly, adapt to changing patient needs, and demonstrate teamwork and dedication to ongoing education.
- National certification in the specialty area.
- Current valid Registered Nurse license at the assigned HMH location by hire date.
- BLS/CPR certification by the American Heart Association within 60 days of hire.
Minimum hourly rate: $45.14. Compensation may vary based on market data, experience, education, certifications, location, and internal equity.
