Description
The Registered Nurse functions as a caregiver for a specific patient population to achieve desired outcomes. This position focuses on expanding knowledge and skills, providing effective direct care as part of the interdisciplinary team to a variety of complex patients. The RN directs and oversees the care provided by other caregivers through delegation, validation, evaluation and coordination, and effectively coordinates patient‑care/unit activities among nursing peers, physicians and support services.
ResponsibilitiesA Registered Nurse at Hackensack Meridian Health delivers nursing practices according to the most up‑to‑date evidence‑based research, nursing policies and standards of care, ensuring safe and excellent individualized patient care.
Assessment- Collects comprehensive data pertinent to the patient’s health or the situation.
- Performs age‑ and disease‑specific assessment and data collection independently in a systematic manner focusing on physiologic, psychological and cognitive status.
- Recognizes clinical and diagnostic status changes in patient status.
- Identifies variables in patient/family education needs based on age, disease and culturally‑specific learning differences.
- Develops a plan of care that prescribes strategies and interventions to attain expected outcomes.
- Formulates age‑ and disease‑specific patient/family‑centered goals and outcomes based on assessment data.
- Coordinates a patient‑centered, prioritized plan of care that encompasses the continuum of care.
- Communicates, coordinates and implements the plan of care.
- Provides ongoing communication with patient/family, nursing colleagues and other members of the health‑care team regarding patient status, rendered care and treatment plans.
- Implements age‑ and disease‑specific care in an organized and timely manner.
- Provides or coordinates patient/family education and ensures understanding.
- Performs according to established protocols in clinical situations, including communication with appropriate persons and documenting events.
- Provides nursing care and treatments that reflect the patient’s needs, advocates appropriately and demonstrates evidence‑based practice and compliance with standards.
- Performs continuous point‑of‑care electronic documentation of assessments, plan‑of‑care, interventions and evaluation of rendered care.
- Administers medications utilizing bar‑scanning and documentation technologies.
- Coordinates multidisciplinary patient discharges to ensure safe transition and continuum of care.
- Evaluates progress toward attainment of outcomes.
- Utilizes critical thinking skills based on nursing knowledge to achieve desired patient outcomes.
- Initiates interventions related to patient’s change in condition and unexpected response to care.
- Identifies ethical issues in the practice setting and brings them to the attention of other team members.
- Recognizes and responds to opportunities to enhance patient satisfaction.
- Acquires and maintains competencies, certifications and continuing education to meet accreditation or licensing requirements.
- Adheres to the American Nurses Association Code for Nurses with interpretive statements.
- Lifts a minimum of 35 lb, pushes or pulls a minimum of 10 lb and stands a minimum of 6 h day.
- Adheres to HMH’s organizational and managerial competencies and standards of behavior.
- Performs other duties and/or projects as assigned.
- Minimum of an associate degree in Nursing or diploma in Nursing.
- BSN in progress and/or willing to acquire within three (3) years of hire (for team members hired on or after November 2019). For those hired after 6/1/2021: BSN within five years of hire; advanced master’s degree in nursing satisfies the BSN requirement.
- Six (6) or more months of relevant nursing experience or active participation in Hackensack Meridian Health’s Nurse Residency Program.
- Strong ability to learn quickly and adapt to changing patient needs, excellent teamwork, pursuit of ongoing education and a passion for delivering high‑quality patient care.
- National certification in area of specialty when eligible.
- Current and valid Registered Nurse license to work at the assigned HMH location by date of hire.
- BLS/CPR certification by the American Heart Association required within 60 days of hire.

The posted rate of pay is a reasonable good‑faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act and does not reflect the full value of our market‑competitive total rewards package.
Minimum rate of $45.14 hourly.
