The Registered Nurse functions as a caregiver for a specific patient population to achieve desired outcomes. The position focuses on expanding knowledge and skills, providing effective direct care as part of an interdisciplinary team to a variety of complex patients. The RN directs and oversees care provided by other caregivers through delegation, validation, evaluation, and coordination, and coordinates patient care/unit activities with physicians and support services.
ResponsibilitiesDelivery of nursing practices according to the most up‑to‑date evidence‑based research, policies, and standards of care. The day‑to‑day responsibilities include: assessment, planning, implementation, and evaluation.
Assessment- Collect comprehensive data pertinent to the patient's health or situation.
- Perform age and disease‑specific assessment data collection independently, focusing on physiologic, psychological, and cognitive status.
- Recognize clinical and diagnostic status changes in patient status.
- Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
- Develop a plan of care prescribing strategies and interventions to attain expected outcomes.
- Formulate age and disease‑specific patient/family‑centered goals and outcomes based on assessment data.
- Coordinate a patient/family‑focused plan of care that is prioritized and encompasses the continuum of care.
- Communicate, coordinate, and implement the plan of care.
- Provide ongoing communication with patient/family, nursing colleagues, and other health‑care team members.
- Implement age and disease‑specific care in an organized and timely manner.
- Coordinate patient/family education and ensure understanding.
- Perform care per established protocols, including documentation.
- Provide nursing care and treatments that reflect the patient's needs, advocate appropriately, and demonstrate evidence‑based practice.
- Document care continuously using point‑of‑care electronic records.
- Administer medication using bar‑scanning technologies for safe delivery.
- Discharge patient with multidisciplinary coordination to ensure safe transition.
- Evaluate progress toward attainment of outcomes.
- Utilize critical thinking skills to achieve desired patient outcomes.
- Initiate interventions related to patient’s change in condition and unexpected response to care.
- Identify ethical issues and bring them to attention of team members.
- Respond to opportunities to enhance patient satisfaction.
- Maintain competencies, certifications, and continuing education as required.
- Adhere to the American Nurses Association Code for Nurses.
- Lift a minimum of 35 lbs., push/pull a minimum of 10 lbs., and stand for a minimum of 6 hours a day.
- Adhere to HMH's organizational and managerial competencies and standards of behavior.
- Other duties and/or projects as assigned.
Education,
Knowledge, Skills and Abilities Required:
- BSN in progress and/or willing to obtain within three years of hire.
- At least six months of relevant nursing experience or active participant with Hackensack Meridian Health’s Nurse Residency Program.
- Rapid learner, strong sense of accountability, teamwork focus, dedication to ongoing education, and passion to deliver high quality patient care.
Skills And Abilities
- National Certification in area of specialty preferred when eligible.
