The NP will evaluate health histories, diagnose and manage care plans, and coordinate with a multidisciplinary team under physician supervision.
Responsibilities include conducting histories and physicals, assessing eligibility for hospice benefits, and documenting care while supporting patients, families, and caregivers in their homes
