Primary Care Assistant-LPN; Non-Exempt

Under the direction of the primary care operations team, the Primary Care Patient Care LPN works directly with Mercy Primary Care Providers to provide safe, timely, effective, efficient, equitable, patient-centered care in a systematic way. The Primary Care Patient Care LPN partners with patients, providers, and other members of the clinical care team to drive quality outcomes for all patients. Duties and responsibilities are performed in a manner consistent with Mercy's mission, values, and Mercy Service Standards.

• Provide safe, timely, effective, efficient, equitable, patient-centered care in collaboration with the care team.
• Work with patients, providers, and other members of the clinical care team to drive quality outcomes.
• Perform duties in alignment with Mercy's mission, values, and Service Standards.

• Education:
  
  Graduate of an accredited practical nursing program.
• Licensure:
  Licensed as an LPN in the state of practice.
• Preferred Experience:
  
  6 months clinical healthcare experience, preferably in a primary care office.

  • Works well as a member of a team and willing to collaborate with all members of the care team.
