Social Worker - Care Manager

Providence – Swedish Edmonds, WA 21601 76th Ave W. Pay range: $45.66 – $69.47 per hour for MSW; $48.39 – $73.64 per hour for LCSW.
Night Shift role. $5,000 Hiring Bonus for eligible external hires that meet the required qualifications and conditions for payment.

The Clinical Social Worker partners with patients, families, and the health care team to address and advocate for patients' social and emotional needs. Responsibilities include psychosocial assessment, treatment planning, therapeutic interventions, discharge planning, crisis intervention, resource referral, and coordination with interdisciplinary teams to improve patient care through effective utilization of health care resources.

• Master's Degree in Social Work from an accredited school.
• Upon hire:
  Washington Clinical Independent Social Worker Associate License.
• 1 year of experience in acute medical or mental healthcare in inpatient or outpatient setting.

• ACM/CCM certification.
