Description:
Candidates must have previous experience in case management.
Experience with patient assessment and care coordination is required.
Candidates will be responsible for conducting initial evaluations and developing care plans to meet patient and caregiver needs.
Must be able to facilitate discharge planning and coordinate services to home care and equipment.
Knowledge of cultural issues and community resources is essential.
Candidates should possess strong documentation skills in relation to plan of care and patient teaching.
Travelers may be expected to float between facilities as needed.
The expectation is to work 1 of 3 major holidays, including Thanksgiving, Christmas, or New Year's.Candidates are required to provide two references covering at least one year of experience within the last three years.
Knowledge of Medicare and Medicaid processes is advantageous.
This position involves working with diverse patient populations, including infants, adults, and geriatrics.
The daily patient census averages between 18-20 patients.
Collaboration with interdisciplinary teams is a key component of this role.
Candidates must comply with client’s policies regarding communication devices necessary for performing assigned duties.
All requested time off must be communicated upfront during the application process.
Inflexibility regarding the contracted schedule may lead to termination of the contract.
