Care Manager RN – Night Shift (Part-Time)

Provides clinical case management functions via a collaborative process that transitions patients to optimal clinical outcomes while assuring safe, cost-effective care. Demonstrates a daily commitment to the department's value of teamwork.

• Manage patient care during night shift, ensuring continuity of care and optimal outcomes
• Collaborate with interdisciplinary team to design and implement care plans
• Monitor patient status, respond to changes, and document interventions
• Communicate updates to day shift team and relevant stakeholders

• Bachelor's Degree in Nursing (BSN) from an accredited school. Candidates without a BSN must actively pursue and attain a BSN within five years of hire. Continued employment beyond five years is contingent upon attainment of the BSN.
• Washington registered nurse license upon hire.
• At least 3 years of current relevant clinical experience.

• Hospital-based case management experience.

Pay range: $45.67 – $81.57 per hour, with potential shift differentials and other compensations (standby/on-call, overtime, premiums, bonus opportunities).
