Optum Care is seeking a Nurse Practitioner or Physician Assistant to provide daily rounding for patients in skilled nursing facilities and long term care settings. You will serve as the primary provider for 1–2 SNFs, managing patients across skilled and long term care levels, with 6–9 encounters daily depending on complexity and facility count.
You will participate in weekly interdisciplinary team meetings and collaborate closely with Extended Care physicians and the patient's primary care team
